Fee: $90 per Team (18-hole Division)

Format: Altenrate Shot

Carts: Golf carts will be avilable for an extra $20 per cart. Please enclose
the cart fee with your entry and mark the box for a cart.
Juniors will not be allowed to drive carts.

Note: There is an option for all Elementary Division players (gradeI K-6)
to play either 9 or 18 holes. The choice rnust be made prior to the
tournament, please mark the box on the entry form to signify the
9-hole division. The starting times for the 9-hole division will begin
at 4:00 off the first tee in consecutive tee tirnes.

Dinner: 7:00pm in the Golf Lodge additional tickets may be purchased
for $17, please include the rnoney with your entry.

Menu: Spaghetti, tossed salad, garlic bread, beverage and dessert
